Preliminary Questionnaire to a Site Visit for Accreditation of a Training Program in Clinical Biochemistry by the CACB

The following information is requested to assist in an efficient and timely on-site accreditation visit. 4. Educational Components of the Training Program

4.1 – Provide a general outline (2-4 pages) describing the philosophy, concept, overall objectives, curriculum outline, etc., for your Clinical Biochemistry Training Program. It is assumed that the Fellowship curriculum will be based on the currently approved CACB syllabus. Include a description of the process used to design the training program. 4.2 – List any formal courses that are available or are offered to candidates in the program. Include details as to staff, topics, and hours of instruction involved. (You may upload documentation for this if you prefer in the Upload section below this question – make sure the question number and topic title are clearly marked on any pages you upload).

4.3 – Indicate opportunities that are available for exposure to the laboratories of cognate disciplines of Hematology, Microbiology, and Molecular diagnostics.

4.4 – What opportunities are available to enhance training in physiology, analytical chemistry, immunology, etc.?

4.5 – What opportunities are available for trainees to gain experience in emerging laboratory technologies?

5. Description of the Practical Component of your Training Program

5.1 – Outline how candidates gain the practical laboratory experience required to meet Syllabus requirements (indicate time spent at the laboratory bench, in each laboratory rotation, and highlight which syllabus topics are supported in each experience).

5.2 – How do trainees gain experience in quality control, quality assurance, and laboratory operations?

5.3 – Outline ways in which trainees are given increasing levels of service responsiblity during the training program.

5.4 – When and to what level are trainees involved in On-Call Clinical Biochemist duties?

5.5 – What opportunities are provided for applied research in the training program? Provide examples of research projects.

5.6 – What opportunities will trainees have to gain teaching experience?

6. Program Evaluation

6.1 – How do you evaluate candidates in the Training Program? Describe the frequency and evaluation process.

6.2 – What criteria do you use to decide if a student has completed the Training Program successfully?

6.3 – If remedial training is needed, do you have a remediation program? Provide an example of a remediation program/plan.

6.4 – How do you evaluate the Training Program itself, the performance of teachers and supervisors, and the adequacy of laboratory training sites?

6.8 – Is there a process for trainees to evaluate individual instructors? How are results returned to faculty members? Provide examples of how this feedback has been actioned on.
